Edgewood Center Pediatrics is excited to announce we will be offering sports physicals to existing patients that have had a well visit in our office within the last year and require an exam for sports on or after April 15th of the previous school year.

If your child’s well visit was on or after April 15th, 2017 a form can be filled out without an appointment.

If your child’s well visit was before April 15th, 2017 and the form(s) require exam to be on or after April 15th of the previous school year you may require a sports physical appointment.

Sports physicals are NOT a complete well exam and do not replace a yearly well visit!

Sport physical appointments are only for filling out required forms.  The exam will include height, weight, blood pressure, heart check, hernia exam (males) and vision exam.  Growth and development/disease management will not be discussed nor will medications be refilled.

If additional testing/procedures are required, an office visit may be billed to your insurance and all  co-pays will be due at time of service.  This will be in addition to the cost of the sports physical!

Sports physicals will be $45 due at the time of service and insurance cannot be billed!

Please download this form and complete the medical history form before your appointment – sports physical form
